Population Health Fellowship – North East and Yorkshire

Applications are invited for the position of Fellow in Population Health, funded by Health Education England.

Three fellowships are available across the North East and Yorkshire in three different host organisations.

Targeted at early to mid-career registered healthcare professionals providing NHS services, as well as wider NHS staff with population health responsibilities, and public sector roles relevant to population health with a leadership responsibility.

Fellowships will commence in September 2023, 2 days per week for 1 year, supported by a complementary education programme.
About population health
The NHS Long-Term Plan places significant emphasis on population health and Health Education England’s Future Doctor programme has identified population health as a core theme. Additionally, the COVID-19 pandemic has further highlighted the importance of population health and health inequalities. Contemporary healthcare is now focused on optimising patient outcomes and reducing health inequalities at population level, and therefore healthcare professionals] across all of healthcare require skills in population health to achieve this.
About Population Health Fellowships
This is an opportunity for healthcare professionals interested in population health and passionate about health inequalities. The aim of the fellowship is to develop a network of professionals from a non-population health background with population health skills to benefit place-based healthcare systems across England.
The fellowship is a one-year part-time programme at two days a week alongside your substantive post. Population Health Fellows will work on a population health related project in a host organisation and will be supported by a blended virtual learning programme. Supervision for the project will be provided.
Reimbursement of your salary element of the participation in the fellowship will be provided to your substantive employer and you must discuss this opportunity with your line manager and educational supervisor (if applicable) of your intentions to apply.
All fellows will commence the programme on 5 September 2023.
Eligibility
Registered healthcare professional providing NHS services (Agenda for Change band 6 and above, or equivalent; dentists-in-training; doctors-in-training post FY2 and their Trust/SAS equivalent). Currently working in an NHS role or other public sector role relevant to population health and with leadership responsibility (Agenda for Change band 6 or above, or equivalent).
Host organisations
There are three hosts across the North East and Yorkshire. These are:
South Tees Hospitals NHS
South Tees Hospitals NHS FT has established a multi-agency strategic group to implement a population health approach to reducing health inequalities. The work programme includes the development of a health inequalities data dashboard to inform action and support the development of local plans to deliver on the Core 20+5, and application of the OHID NE health inequalities impact assessment toolkit to guide priorities and policy development. The fellow will work with a Public Health Consultant across this initiative, helping to develop a deeper understanding of current challenges across the region and identify priorities for population health interventions.
NHS Wakefield West Yorkshire ICS
NHS Wakefield West Yorkshire ICS has a universal interest in improving health for all with a focus on those who experience the poorest health outcomes and has developed a reputation for developing a wide range of innovative population health interventions. This fellowship would support the selected fellow to contribute to a leadership project in one of our programme areas including; prevention, health inequalities, climate change, violence reduction, health and housing, and population health management. Scope exists to design new discrete projects that could be delivered within the fellowship timescale and in line with the interests of the fellow.
Leeds Teaching Hospitals Trust
LTHT has worked with multi-agency partners over recent years to deliver population health management across Leeds with the aim of developing local care partnerships that can have a genuine impact on health inequalities. Current priorities are focused on the further development of NHS Providers, including large institutions and GP Partners, to work across communities to deliver integrated person-centred services. The fellow would have the unique opportunity of being hosted within an acute trust but working across a provider system to support development of the building blocks for provider-led population health delivery.
