Core Psychiatry Training Course
The Core Psychiatry Training Course (previously known as the MRCPsych Course) was commissioned by Health Education England working across Yorkshire and the Humber in August 2014. The course is delivered jointly by The University of Leeds and Sheffield Health and Social Care NHS Foundation Trust for Core Psychiatry and Broad Based Trainees across the Yorkshire and Humber region.
The course consists of teaching sessions and lectures for each year level of training, which junior doctors will undertake in sequence. The course aims to provide a framework of knowledge and skills relevant to prepare the doctor both for the MRCPsych examinations and for the clinical demands of higher training
Teaching takes place one day per fortnight for Year 1 and 2 trainees, and half a day per fortnight for Year 3 trainees.
Course Centres
Sheffield Health and Social Care NHS Foundation Trust (SHSC) collaborate with psychiatrists and academics from the University of Sheffield, Derbyshire Healthcare NHS Foundation Trust, Nottinghamshire Healthcare NHS Trust, Rotherham Doncaster and South Humber NHS Foundation Trust, Sheffield Children’s Hospital NHS Foundation Trust, Sheffield Teaching Hospitals NHS Foundation Trust, South West Yorkshire Partnership NHS Foundation Trust, Sheffield Hallam University, the University of York and the University of Central Lancashire to provide teaching in the south of the region.
The University of Leeds collaborate with psychiatrists and academics from Leeds and York Partnership Foundation NHS Trust, Leeds Community Healthcare NHS Trust, South West Yorkshire Partnership NHS Foundation Trust, Bradford District NHS Care Trust, Humber NHS Foundation Trust, and the Yorkshire part of Tees, Esk and Wear Valleys NHS Foundation Trust to provide teaching in the north, west and east of the region.
Recognising and Assessing Medical Problems in Psychiatric Settings (RAMMPS)
Recognising and Assessing Medical Problems in Psychiatric Settings (RAMPPS) is an innovative multi professional course using clinical simulation. Aimed at teams working and training together it focuses on recognising the deteriorating patient and understanding the human factors important to delivering good care in these circumstances. It uses structured frameworks in safely managing patient outcomes.
The inter-relatedness of physical and mental health has received much attention in recent years with good reason. Patients suffering from serious mental health problems have more physical health problems and a greater morbidity and mortality rate than the general population: A threefold excess mortality and a life expectancy of around twenty years less. Since 2012 Health Education Yorkshire and the Humber has been supporting our innovative multi-disciplinary course RAMPPS. The course is explicitly multidisciplinary focusing on medical, nursing and support staff as well as other professional groups. The scenarios use a high fidelity approach through different hybrid simulation modalities such as full body manikins, part task trainers and simulated patients. This course is being delivered in a number of quality assured clinical skills and simulation centres across our region

eLearning Module
The RAMPPS course is supported by our interactive RAMPSS e-learning module, which takes about 15 minutes to complete. The e-learning module is suitable for all staff in mental health settings to help improve skills in recognising the deteriorating patient using an ABCDE approach, AVPU and the SBARD communication tool.
The module can be accessed via the eLearning for Health site.

Publications
The RAMPPS work has been presented at various local, national and international conferences and summarised in an article in the Health Informatics Journal explaining how RAMPPS meets the Technology Enhanced Learning agenda, copies of the published work can be found below.
RAMPPS has been chosen as a case study in the Improving the physical health of people with serious mental illness. A practical toolkit. and in MINDSet.
RAMPPS has a standardised approach to training faculty to provide consistency for all RAMPPS courses. There is a minimum slide set for training attached here and a "gold standard" facilitator pathway - please download this from the bottom of the page.
Calderdale and Kirklees Core Training
Teaching programme
The educational programme takes place on Wednesday mornings across the Trust and includes the following:
Journal Club/Case Conference - held on the second Wednesday of the month for all medics in Dewsbury, Halifax and Huddersfield. Trainees are expected to present a case conference and also review a journal article.
Clinical Interview Skills Training - takes place on the 2nd wednesday of the month and is for core and GP Trainees.
Audit - takes place on the 3rd Wednesday of the month for all medics in Dewsbury, Halifax and Huddersfield. This is an opportunity for trainees to present their audit findings.
Psychotherapy teaching - Balint groups are held on the 1st, 2nd and 3rd Wednesdays and a teaching session takes place on the 1st Wednesday of the month. Supervision takes place every month.
Junior Doctor peer teaching - by rota core Trainees deliver a teaching session to their peers on their chosen topic. This takes place on the 1st Wednesday of the month.
Joint Academic Psychiatric Seminars (JAPS) – take place on the 4th Wednesday of the month with external and internal speakers. These seminars are attended by all Trust medical staff.
ECT training - this is a 3-step process leading to trainees being signed off as per the Royal College Guidelines. Our ECT suites in Wakefield and Calderdale are both accredited with excellence by ECTAS.
Medical students are attached on a regular basis in each locality and core Trainees are encouraged to participate in their teaching.
Exam preparation
- Mock exams are offered for papers 1, 2 and 3
- Mock CASCs examinations are carried out twice a year and although they are offered to candidates throughout Health Education England working across Yorkshire and the Humber they are free to candidates working in South West Yorkshire Partnership NHS Foundation Trust.

East Riding Core Training
Teaching Programme
In house teaching is held on Wednesday mornings. This is usually held at the Castle Hill Lecture Theatre. The teaching programme includes:

Mock exams organised with our neighbouring rotation, North Yorkshire. These are held some weeks before the exams alternating in Hull or York.
CASC Training
Consultants and specialist registrars will regularly see trainees for mock exams outside the Wednesday morning training.
Deliberate Self Harm assessments in accordance with Royal College Guidelines.
Psychotherapy Training
The amount of psychotherapy training required by the royal College of Psychiatrists is increasing and is mandatory. Psychotherapy training is led and coordinated by Dr M Stephenson, Consultant Psychiatrist in Psychotherapy.

North Yorkshire Core Training
Case presentations and Journal Clubs are provided every Wednesday morning at York District Hospital, often interspersed with guest speaker presentations. Wednesday teaching is followed by Psychotherapy supervision - two Balint groups for CT1 doctors and two CBT and one psychotherapy case based discussion groups for more senior trainees.
There is also a full day annual symposium in the Autumn – this will be held in November in 2014 and further details will be displayed here.

South Yorkshire Core Training
Each of the five trusts within the scheme provides an educational programme for core trainees. The local opportunities include: case conferences, journal clubs, supervision groups, taster sessions in psychiatric sub-specialities, clinical governance and audit meetings.
A regional case conference and journal club takes place in Sheffield once a month. there is an active trainee committee and regular trainee representation at the Speciality Education and MRCPsych committees.
Trainees are encouraged to teach medical students from the University of Sheffield and they are encouraged to develop research interests through the University of Sheffield Academic Clinical Psychiatry department.
There is a priority to develop clinical simulation training across the scheme and to this end trainees are expected to attend formative assessments in communication skills (FACS) at CT1 level, management of physical healthcare emergencies in psychiatric settings (RAMMPS) at CT2-3 level, and management training sessions at all CT levels.
The development and assessment of psychotherapy competancies is overseen by a dedicated psychotherapy tutor and there is a structured programme of case-based discussion, short case and long case experience.

Intellectual Disability Higher Training
There is a Y&H wide academic programme which consists of monthly meetings open to consultants and SAS doctors as well as higher trainees, monthly higher trainee tutorials and a monthly Balint group for higher trainees.

West Yorkshire Core Training
The Educational programme in Leeds takes place each Wednesday across the Trust and includes the following:
Journal Club / Case Conference (Leeds): Trainees are expected to review a journal article and present a case conference
Audit (Leeds): Held the last Wednesday of the month. Trainees are expected to present an audit and findings which they have been involved with
Guest Speakers (Leeds): Internal guest speakers are invited to attend the teaching programme
Psychotherapy teaching (Leeds): VTS/Core Trainees: Weekly Balint groups are held on Thursday lunchtimes. This time is agreed and protected. The Trainee is invited to talk about a patient or situation at work which will be considered from psychodynamic perspective. Core Tarinees 1's must spend a year in one of the groups before they are eligiible to join a supervision group to take on a case in therapy (Dynamic, CBT). Further details can be obtained from from Dr A Cooper, Consultant Psychiatrist.
F1 & 2's: Weekly Balint Group held on Wednesday's 4-5pm. This time is protected. Further details will be provided by Dr Fitzgerald.
Undergraduate Teaching Experience (Leeds): All core trainees in Psychiatry and GP Trainees have protected time to deliver the Campus to Clinic teachings for second year medical students.
- CT1/GP Trainee: 4 hours protected teaching time in a 6 month period.
- CT2: 6 hours protected teaching time in 6 month period
- CT3: 8 hours protected teaching time in a 6 month period
ECT Training (Leeds): ECT Training is organised and training dates are circulated via the Medical Education Centre.
Junior Doctors Committee (Leeds): Meets the last Monday of the month. All Core Trainees are expected to attend.
Wakefield Locality
The educational Programme in Wakefield takes place each Wednesday morning - the meetings are held in the Learning & Development Centre, Fieldhead.
Journal club/case conference (Wakefield): Trainees are expected to review a journal article and present a case conference.
Audit (Wakefield): Held the second Wednesday of the month. Trainees are expected to carry out an audit (with their team/colleagues) and present the findings at one of the monthly meetings.
JAPS (Joint Academic Psychiatric Seminars) (Wakefield): Wakefield is part of SWYPFT and covers the East of the area, Barnsley to the South and Calderdale and Kirklees to the West. The JAPS meetings are held the fourth Wednesday of the month at the Al Hikmah Centre, in Batley, where medical staff Trustwide are invited to attend. The speakers can be either internal members of staff or those who work externally for other organisations.
Psychotherapy teaching (Wakefield): VTS/Core Trainees: Weekly CBDG group is held on Wednesday lunchtime - after the teaching programme lunch. This time is agreed and protected. The group is run by Dr Saju.
F1 & 2's: Weekly Balint Group on Wednesday 3:45 - 4:45pm run by Dr Chris Douglas. This time is protected.

General Adult Higher Training
General Adult Higher Training for East, North and West Yorkshire
Higher trainees attend a monthly training programme with external speakers, addressing clinical, academic and managerial themes.
Trainees are collectively encouraged to sculpt their own training programme and this maximises relevance to core competencies.
Management and Leadership Programme for Senior Registrars and Doctors
A course aimed at senior registrars who want to develop their knowledge and skills in leadership and management.

Child & Adolescent Higher Training
North, West & East Yorkshire
There is an academic training programme in the North, East & West taking place in Leeds every Thursday afternoon during term time coordinated by Drs Jon Barrett and Siobhan Smart. Trainees play a full role in presenting and preparing for these sessions which are a mix of activities in a rolling 3 yearly programme supporting the child and adoloscent curriculum. This programme takes 50% of study leave for a year.
South Yorkshire
Whilst exciting plans to amalgamate programmes with the North, West and East are finalised, Higher Trainees in the South attend a weekly training programme addressing clinical, academic and managerial themes.
Trainees are actively encouraged to participate in and contribute to their own training programme. The Higher Training programme is held every Tuesday between the hours of 13.30 and 17.00 and is split into two component parts.
13.30 - 15.30 - Covering a range of topics from CPD to Psychotherapy or opportunities to meet with the Deputy TPD
15.30 - 17.00 - Will alternate sessions between Case conferences, trainee committee or journal club.
Management Training
Formal training
Higher trainees are encouraged to develop their Management and Leadership skills through formal (generic) courses available via Health Education Yorkshire and the Humber. In addition to the highly popular ‘Management and Leadership’ module, there are courses available on ‘Medical Ethics and Legal issues’, ‘CV writing’, ‘Skills of Career guidance and Mentoring’, ‘Making the transition to Consultant’ and ‘Recruitment and Selection interview’. Many have enrolled on the online LeAD course endorsed by the Academy of Royal Colleges, tailored around the Medical Leadership Competency Framework. Some trainees have chosen to pursue self-funded MBA courses at local Universities, and other comparable self-directed approaches.
Apprenticeships
In addition to formal training, there are frequent opportunities for supervised apprenticeships in discreet areas of medical management. Trainees are encouraged to take on small, discreet, time limited projects to manage, ideally pairing up with a colleague, and allowing for deliverables within a 12 month time span. Recent examples include an externally funded evaluation project of transformation of care pathways in Leeds (the Transformation project) which has been offered to higher trainees under the supervision of local consultants, Dr Nick Venters and Dr Guy Brookes. Another trainee spent two years developing a local MARSIPAN group - bringing together physicians, eating disorder psychiatrists and liaison psychiatrists, including drafting of terms of reference and subsequent evaluation.
As members of the Higher Trainee Committee, which meets monthly, trainees are also encouraged to pursue membership and active participation of various committees (for example, the Local Negotiating Committees, School Management Committee, etc) across the region. More recently, a trainee has been elected as a representative on the Psychiatric Trainees Committee at the Royal College of Psychiatrists.
