The West Yorkshire training programme offers comprehensive Core and Specialist Training programmes providing a mixture of busy District General and Tertiary referral centre experience. On completion of their training programme our trainees are capable of dealing with anything a busy hospital can throw at them!
Core rotations are based around the following hospitals:
- Bradford
- Airedale General
- Mid Yorks Hospitals (consisting of Pinderfields (Wakefield), Dewsbury and Pontefract Hospitals)
- Calderdale and Huddesfield Trust (consisting of Calderdale Royal (Halifax) and Huddersfield Royal Infirmary)
- Harrogate
- Leeds Teaching Hospitals (consisting of Leeds General Infirmary (LGI) and St James's University Hospital (SJUH))
Trainees may also rotate to smaller hospitals within these Trusts.
Core Anaesthetic training would consist of 2 years, typically 1 year in a larger hospital and 1 year in a smaller hospital as decided by the Core Training Programme Directors. Those doing ACCS Anaesthesia spend 3 years as a core trainee (2 years anaesthesia, 1 year acute/emergency medicine) organised by the ACCS Training Programme Director.
Specialist (Higher or ST) Anaesthesia rotations are centred around the "Leeds Hub". These rotations can include all the above Trust, as well as the following:
- York
- Hull
- Doncaster
- Rotherham
- Sheffield Teaching Hospitals
The rotations are optimised to ensure trainees do not have to commute the width of Yorkshire during their rotation. Typically a larger proportion of time is spent in the teaching hospitals for speciality training especially as the concept of spiral learning means that the speciality blocks have to be done twice.
Once intermediate and higher level training are completed, trainees choose an area of advanced level practice and prepare for Consultant Practice.
Advanced Level Training can be undertaken in a number of the region's hosptials. The following document outlines the modules on offer and some tips on how to prepare for Advanced Training

Hospital Website Airedale Trust
High performing Foundation Trust, consistently in the top 10 nationally for quality of care. Opened in 1970, covering a population of 200 000 people over an area of more than 700 square miles of West and North Yorkshire and East Lancashire. Currently has in the region of 300 inpatient beds.
Location
On the B6265 between Keighley and Skipton, at the “Gateway to the Dales”. In addition to Bradford and Leeds, there are good shopping centres and cultural facilities at Ilkley (7 miles), Skipton (5 miles) and Harrogate (20 miles). Steeton train station is less than a mile from the hospital with direct links to Leeds, Bradford, Skipton and further afield.
Clinical Experience
There are 7 operating theatres in one self contained unit, plus a satellite treatment suite. Surgery undertaken includes general, orthopaedic, trauma, urology, gynaecology, obstetrics, day case maxillofacial and ENT, children’s exodontia and cataract surgery. Plans for two new additional theatres are in development.
There is a fully staffed ED and AAU. There is an active Consultant led acute pain service. The obstetric unit has 2600 deliveries a year.
The 10-bedded Critical Care Unit provides ITU/HDU flexibly with outreach services and facilities for multi-organ support. Due to its size it does not currently provide basic or intermediate level ICU training.

The Core trainees work a fully EWTD compliant 1 in 8 full shift rota.
Teaching
Weekly journal club. Fortnightly CT tutorials on a Wednesday afternoon.
Trainees have their own office and there are computers readily available along with secretarial assistance. The Post-Graduate Education Centre has a library and computers with internet access. There is a recently opened Clinical Skills Centre offering a wide variety of simulation courses. There is plenty of opportunity for anaesthetic trainees to be involved in the formal teaching programme for FY1 and 2’s and medical students.
Trainees are encouraged to attend the Yorkshire Primary FRCA Course.

This busy Foundation Trust has had Teaching Hospital status since 2003. Services are split between the main hospital at Bradford Royal Infirmary and the smaller peripheral hospital at St Lukes, with the vast majority of our anaesthetic work being undertaken at Bradford Royal Infirmary. There are approximately 1200 beds between the two sites providing acute hospital care services for Bradford and neighbouring communities.
Rota
We run 4 full shift rota tiers i.e. Senior Registrar, Obstetrics, Acute Theatres and Intensive Care. These are EWTD compliant and currently banded as 1A.
Transport
Bradford Royal Infirmary is on the outskirts of Bradford located approximately 2.5 miles from Bradford Interchange Rail Station. Buses are available from the Interchange departing from stand A20 - Bus No. 617 or 618.
Clinical Experience
Bradford Royal Infirmary has 20 theatres delivering approximately 20 000 anaesthetics per year allowing a wealth of potential clinical experience. We provide anaesthesia for multiple surgical specialties including Vascular, Orthopaedics and Trauma, Upper GI and Bariatric surgery and Complex Maxillofacial Surgery amongst other specialties. Additionally, we provide a service for CT/MRI and anaesthetic-run Pre-Assessment and Sleep Clinics as well as supporting a busy Accident and Emergency department.
Intensive Care Medicine
Bradford Royal Infirmary has a new 16 bedded adult ICU/HDU. Cover is provided by 2 consultants per day, both dedicated intensivist, one working for the entire week. We have a full time Intensive Care Research Nurse.
From a training perspective, the unit is accredited for all levels of ICM training including Advanced level training and is also a centre for the European Diploma of Intensive Care Medicine.

Pain Management
St Lukes Hospital is home to our busy Pain Management Centre which is run by 5 Consultants with a specialist interest in Pain Medicine, supported by two full-time Clinical Nurse Specialists and a Research Nurse in Chronic Pain Management. The service is largely out-patient based offering both conservative and interventional management to patients. In addition the service offers a Pain Rehabilitation Programme. As such, the service offers an excellent resource for pain training, and Bradford Royal Infirmary is accredited for all levels of Pain Training including Advanced Level Training.

Sleep Medicine
This continually expanding service is run by 3 Consultants with an interest in Sleep Medicine working with a Respiratory Physician and Respiratory Nurse Specialist. Approximately 600 referrals are received per year linking in with both Bradford’s ENT department and Bradford’s Orthodontic Department. The service also runs an efficient CPAP service allowing remote monitoring of patient compliance therefore minimising hospital visits and enhancing patient experience.

Obstetric Anaesthesia
Bradford Royal Infirmary sees approximately 6000 deliveries per year on its busy maternity unit. There are 6 Obstetric Anaesthetic Consultants providing consultant cover between 8am and 6pm from Monday to Friday. Out of hours, anaesthetic cover is provided by a resident trainee with support from an On-Call Consultant. Bradford Royal Infirmary is accredited for all levels of training in Obstetric Anaesthesia, including Advanced.

Pre-Assessment
We run an Anaesthetic Pre-Assessment Clinic twice a week at St Lukes Hospital allowing the early identification of patients potentially at risk of peri-operative complications. This allows stratification of that risk and pre-optimisation of the patient as appropriate. Part of the risk stratification includes targeted investigations including an on-site Cardio-Pulmonary Exercise testing service for appropriate patients. This is an increasingly busy clinic currently supported by 3 Consultant Anaesthetists.

Anaesthesia for Difficult Airways
Fibreoptic Intubation is now a mandatory skill to be taught during Intermediate and Higher Level Anaesthetic Training. At Bradford we see a large proportion of patients with difficult airways both on an elective and on an emergency basis. Whilst we teach fibreoptic skills on a clinical basis, we also currently run 2 very practical Awake Fibreoptic Intubation Courses per year in collaboration with Leeds Teaching Hospitals.

Regional Anaesthesia
With advances in ultrasound technology, regional anaesthesia has progressed rapidly in recent years and has become almost exclusively ultrasound guided. It is now used routinely for a number of procedures to provide both surgical anaesthesia and post-operative analgesia either by continuous infusion, or as a single shot injection with excellent results. The large number of operations involving regional anaesthesia within Bradford provides a wide scope for teaching/learning experiences.

Research
Research is recognised as a high priority by the Trust and, as such, receives support from the Bradford Institute for Health Research. Several members of the department are involved in local research projects. In addition, there are an expanding number of National Institute for Health Research portfolio studies to which the department recruits. These fall within the areas of Critical Care, Anaesthesia, Perioperative Medicine and Pain Management. Trainee involvement in on-going projects is strongly encouraged.

Bradford Anaesthetic Department has many years’ experience in supporting trainees facing the Primary or Final FRCA. The department also runs a mock Regional SOE/OSCE day for Primary FRCA every year.

Teaching
There is a weekly teaching programme which all Pre-Fellowship trainees are expected to attend and present at. This is currently co-ordinated by Dr Caz Farrow.

There is internet access for trainees to undertake e-learning within the department as well as access to material within the hospital library.
There is also a comprehensive novice programme co-ordinated by Dr Ali.
Training
In line with the Royal College of Anaesthetists Curriculum 2010, our training is largely modular based particularly for those trainees who have achieved their Basic Level Certificate in Anaesthesia. At present we are able to deliver training for the Basic Level Certificate, most Intermediate sub-specialties (with the exception of the sub-specialties of Cardiac and Neuro-anaesthesia), General Higher modules as well as Advanced Training in Intensive Care Medicine and Chronic Pain Medicine as stated above. Basic, Intermediate and Advanced Training Modules are overseen by consultants in the department with an interest in each specialty.

Simulation
Bradford Simulation Centre and Technical Skills Lab opened in May 2011. The centre includes a purpose built mock theatre/resus area and 4 bedded ward. There are several medium-fidelity manikins including SimMan 3G, SimBaby and child and obstetric manikins. Courses include Transfer Training, Obstetric Anaesthesia, Non-technical skills training and Novice anaesthetic training. Information on courses can be obtained from Tracey.Williams1@bthft.nhs.uk
The technical skills lab houses three Ultrasound machines and several “Blue phantom” models for training in USG IV access, nerve blocks and thoracocentesis.

Clinical Governance
In recent years, Clinical Governance has become increasingly recognised as being essential to ensure that standards of patient care are maintained and continually improved upon. As such, each hospital department undertakes regular meetings as an opportunity to reflect on the seven pillars of Clinical Governance. We expect all members of the department to participate regularly in Clinical Governance meetings which are held on a monthly basis including the presentation of Clinical Audit. The Calderdale Royal Hospital is situated 1.5 miles from Halifax town centre and is a PFI hospital first opened in 2001. Huddersfield Royal Infirmary is situated 3miles from Huddersfield town centre and was first opened in 1965. The hospitals are about 4.5 miles apart.
The trust is spread across two sites; Calderdale Royal Hospital in Halifax and Huddersfield Royal Infirmary in Huddersfield and the services configured across both sites. Many of the staff work cross site and we hope you will have
the opportunity to do so too depending on your training requirements.
The Trust serves a very large and diverse population of 435000 people and have 810 in patient beds. The department is recognised as being very friendly and keen to support all of our trainees. We offer all the main DGH type specialities and have a well organised teaching programme for trainees with a new management training programme also under development for our more senior trainees.
Calderdale Royal Hospital
The services located on the Calderdale site include elective Orthopaedics, Obstetrics, Opthalmics, ENT, Paediatrics, General Medicine and Cardiology. CRH is a PFI built hospital replacing the two older Halifax hospitals in 2001.Situated
within reasonable walking distance to the town centre and its train station as well as the M62 for easy access to both Leeds and Manchester.
We have 8 theatres located on level 3 of the hospital with a mix of specialities, the main exceptions are major GI (upper and lower), Vascular, urology and acute general and trauma theatres which are located on the HRI site.
The theatres surround a central “recovery area” and have an admissions area where all inpatients are admitted and can be seen prior to their operations. The day stay unit is also adjacent although there are no separate day stay lists and all
operations take place within the main theatres.
The Anaesthetic department is within the theatre complex and contains the offices of the consultants, secretaries, a small coffee room for anaesthetic staff as well as a seminar room for meetings and teaching.
The intensive care unit is on the 3rd level and although has the physical capacity of 8 beds, is currently only staffed for 5. Admitting mainly medical patients we have full ITU facilities and haemofiltration available. Labour ward is on level 2 just beneath main theatres with adjacent ante and post natal wards. Elective patients are admitted to ward 1D on ground floor.
Huddersfield Royal Infirmary
Huddersfield theatres are situated on the ground floor of the hospital next door to the intensive care unit and close to the main reception.
Daily acutes/CEPOD lists and separate Trauma lists as well as major GI surgery, urology and vascular surgery all take place at Huddersfield. Ward 14 is where the majority of elective general surgical patients are admitted with the surgical
assessment unit and intensive care within the same block.The “pink room” is the designated trainee room and houses 2 computers with internet access and a projector for teaching.
The anaesthetic secretaries are situated just opposite the theatre complex on the main corridor back to reception. Huddersfield takes all acute surgical admissions (exception of ENT,Eyes and gynae) and will also receive all trauma.
The emergency department is one level below the main theatres. There is also a dedicated day surgery unit on the lower ground floor level.

Harrogate District Hospital is a busy DGH situated in an attractive spa town with good transport links to Leeds (15 miles) and York (20 miles).
Clinical experience
The main theatre suite has 5 operating theatres, including a dedicated 24-hour emergency theatre, and a 5-bay recovery area and lies adjacent to the ITU/HDU. The inpatient surgery undertaken includes – General, Orthopaedic and Trauma, Gynaecology, Breast, Urology, Ophthalmology.
There are 3 DSU theatres. These provide, in addition to the above specialties, both adult and paediatric ENT, Dental and Maxillofacial surgery.
The 10-bedded Critical Therapy Unit provides ITU /HDU and outreach services with facilities for multi-organ support.
Our Obstetric unit, with over 2000 deliveries per year, has 7 delivery rooms and a dedicated theatre and provides a 24- hour obstetric analgesia and anaesthesia service.
Multidisciplinary acute and chronic pain services are provided.
Rota
Trainees work a 1:7 EWTD compliant rota (48 hours per week). Currently under review (Aug 2017).
Teaching
The department places a great deal of emphasis upon education resulting in a good exam success rate. There is protected teaching time with tutorials and exam preparation one morning per week with opportunities for trainees to present and lead teaching.
Multidisciplinary ITU teaching takes place including a journal club.
Within the department there is a seminar room with journals and a small library in addition to internet access. The PGED also provides excellent learning facilities, including a simulation centre.
Senior trainees have the opportunity to gain management experience and to attend the HELM course run by the Trust.
Monthly audit meetings take place, some of which are multidisciplinary, and there bret.claxton@hee.nhs.ukare opportunities to take part in both local and national audits and research.

The General Infirmary is in the centre of the city, placed between the main shopping and commerce areas and the University. As part of the larger LTHT, where clinical services are constantly moving between sites, akin to Brownian motion, the LGI is home to Neurosurgery, Cardiac surgery, Paediatric surgery, Vascular surgery with all the attendant radiology and medical diagnostic services. Leeds is currently expected to become a level 1 Major trauma centre. In the department of anaesthesia we currently have 87 Consultants, 2 Associate specialists, 2 Staff grades and 48 Trainees. The office accommodation has 4 secretaries, a seminar room with Smart board, a sitting room, a trainee computer room and a television in a box awaiting an antenna.

Trainees
As a general rule, ST3 and ST6 years will be in the LGI, allowing the trainee to complete the spiral learning as described by the 2010 curriculum. There will also be a smattering of ST4/5 who may still need to complete individual modules and ST7 who are undertaking a specific Advanced module available in the LGI.
St James's Hospital (aka Jimmy’s)
'Jimmy’s' and the Leeds General Infirmary are each among the largest teaching hospitals in Northern Europe. They are both part of the Leeds Teaching Hospitals NHS Trust which itself is one of the largest Trusts in the UK.
Jimmy’s, just North East of the city centre, started out as a workhouse for the poor in 1845 but by the end of the 19th century all the component buildings had become used for medical care of the poor. St James’ Hospital is named after Dr James Allen, a former medical superintendent and Sir James Ford who oversaw the conversion from workhouse to hospital. Some of our trainees still see Jimmy’s as a workhouse!
Jimmy’s has hit the limelight over the years for various reasons; the Yorkshire TV documentary “Jimmy’s” televised between 1987-1994 followed the fate of junior medical staff, and more recently for performing the first NHS live donor liver transplant in 2007.
From an Anaesthetic perspective the Department has roughly 45 Consultants and 36 trainees (predominantly ST’s with 4 or 5 CT1’s at any one time). We provide Anaesthesia for a wide variety of adult surgical specialities including transplants (livers and kidneys), Thoracics, Upper and Lower GI Surgery, Bariatrics, major Urology and Gynae Surgery to name but a few. There has been huge investment in and centralisation of cancer services on the St James’ site with the building of the state of the art Bexley wing. Being a tertiary referral centre, there is no shortage of challenging cases to test your clinical skills and boost your logbook.
There are approximately 34 critical care beds (16 level 3), the majority of which are looked after by Anaesthesia. Trainees allocated to ICU will be on a separate on call rota and disappear from Anaesthesia usually for 3 months at a time. There are 3 trainees on the ground at any one time in ICU (sometimes more during the day) with 2 consultants and between existing patients, outreach and ward/A+E referrals there is plenty of work to go round.
The Delivery Suite has just under 4000 deliveries per annum with an epidural rate approaching 50%. As a tertiary referral centre with vascular X ray many complex pregnancies (eg placenta acretia) are managed at Jimmies. The complex cardiac cases are referred to the Leeds General Infirmary where there is another obstetric unit of similar dimensions. Plans are afoot to merge the 2 units onto one “superunit”.
In summary Jimmies offers excellent anaesthetic experience in a friendly department together with sub specialist training in thoracics, transplants, obstetrics, chronic pain, intensive care medicine, vascular access and major abdominal surgery. Other core modules are available at the Leeds General Infirmary.

The hospital is minutes away from the M62 and M1 and is within easy reach of Leeds. It is about 20 minutes walk, or a short bus ride, away from Wakefield Westgate railway station.
Clinical Experience
The hospital is a new build large district general hospital, opened in February 2011. The anaesthetic department has expanded dramatically over the last few years. We now have 50 consultants, all with special interests that they wish to share with trainee anaesthetists. Training is particularly strong in difficult airway management and in anaesthesia for plastic and orthopaedic surgery under regional blocks. We have a 20 bedded consultant led critical care unit, including two Burns Intensive Care beds as part of the Regional Burns Unit providing services to Yorkshire and Lincolnshire. The unit is accredited for all stages of Intensive Care training. We also offer dedicated 2 week burns attachments for senior Anaesthetic and ICM trainees from across the Yorkshire and Humber deanery.
All trainees have dedicated educational supervisors. We are able to deliver all the current curricular basic training blocks and a large number of intermediate and advanced training blocks.
Intermediate UoT delivered at MYH: General Duties, Intensive Care, Paediatrics, Pain, Ophthalmic, Plastics/Burns and obstetrics
Higher UoT delivered at MYH: General duties, Intensive care, paediatrics, Pain, Ophthalmic, Plastics/burns and obstetrics.
Advanced UoT delivered at MYH: Airway, General/urology/gynaecology, day surgery, sedation, orthopaedics, regional, obstetrics, peri-operative medicine, Intensive care and Plastics/burns.
The educational leads aim to be approachable and contactable and we are very happy to be asked questions about training by current or prospective anaesthetic doctors.
The canteen has a good range and is open daily. Car parking can be a problem but if you get there in time for morning lists to start then there shouldn’t be an issue. Trainees will also be allocated to some lists at Pontefract or Dewsbury Hospital. Lists at Pontefract are mainly day surgery and give trainees the opportunity to learn more about this increasingly important sub-specialty. Lists at Dewsbury include both major surgery and day case surgery.
Rota
There are currently five fully working time regulation compliant rotas at Pinderfields. First on acutes is normally staffed by the least experienced doctor and covers acute theatres supported by the second on, who is the most experienced on-site doctor. The second-on also provides support to the critical care and obstetric anaesthesia resident doctors who provide the services that you may be able to deduce from their titles. There is also a novice on call rota on ICU and a second on Obstetric rota.
There are two dedicated on-call consultant anaesthetists available at all times, one covering critical care, the other covering acute anaesthesia and obstetric anaesthesia.
Teaching
There is a new comprehensive teaching package for novice anaesthetists.
Weekly teaching takes place on a Wednesday AM. There are a number of interesting departmental meetings and teaching sessions that trainees are welcome to attend including; weekly Intensive Care lunchtime teaching, weekly lunchtime interesting case discussions, a regional anaesthesia group that meet to demonstrate and practice regional blocks, a weekly ICU Journal Club, and of course the monthly departmental audit meeting. There is also a bi-monthly joint speciality academic meeting, presently run with Respiratory medicine and Cardiology.
We also offer intensive support prior to the viva SOE/OSCEs for both Primary and Final, run by an enthusiastic cohort of consultants, and have had excellent pass rates for these over the past few years.
There is an excellent postgraduate centre with full library facilities
